Q I have a hiatus hernia and as a result I suffer from gastric acid reflux, which is kept in check by esomeprazole tablets every 48 hours. I have a permanent dry cough and am worried my vocal cords may be damaged.

A Acid reflux is a common gastrointestinal disorder with the usual symptom being indigestion or “heartburn”. Long-term medical treatment for reflux is safe but you may find better relief from your specific symptoms by taking your prescribed tablet daily.
Chronic acid reflux can cause hoarseness, or polyps on the vocal cords. If symptoms persist despite treatment then keyhole surgery is possible on the hernia itself, but is considered only after all other options have failed.
Q Two years ago I had episodes of sciatica down my left leg that left me with some muscle weakness. My neurosurgeon said I had probably also slipped a disc. Obviously, my sciatic nerve has been affected, but do the damaged nerve fibres regrow?

A A slipped disc causes pressure on the sciatic nerve rather than fraying or splitting it. As a result, there is no need for your nerve fibres to grow back. I suspect you have fully recovered anatomically now but are understandably wary of retriggering your problems.
Provided you avoid sudden bending or twisting movements, exercise at a level that rebuilds your muscles. Swimming, cycling and Pilates will all help.
Q I suffered two subdural haematomas 11 years ago and since then have endured migraine-like symptoms on flights. My GP offered to prescribe painkillers. Is there anything else I could take?

A A subdural haematoma is a blood clot occurring between the outer two membranes of the brain. Even after treatment, “hangover” symptoms such as headaches can occur. In your case, these are apparently triggered by alterations in air pressure. Some people report benefit from taking feverfew, a natural treatment, daily for several weeks prior to flying. Check with your doctor first.
